Clinical Study and Identification of Candida Species in the Patients With Diaper Dermatitis




Abstract
Authors performed clinical and mycological study for diaper dermatitis on 50 patients who had visited Department of Dermatology, University and Yongsan Hospital, College of Medicine, Chung Ang University from May, 1990 to October, 1990. The patients were examined and graded according to cutaneous manifestations. We carried out KOH scraping and culture in Sabouraud¢¥s glucose agar (SGA) media at the inguinal, perianal area and buttock. 75 strains of candida species were isolated and identified by culture in Corn meal-Tween 80 media and sugar assimilation and fermentation tests using API 20C AUX strip system.
The results were as follows:
1) The ratio of male to female was 1.2 : 1 (27 : 23) and mean age was 8.3 month.
2) The peak incidence occurred in the 6 to 12 month age group.
3) The most common condition associated with diaper dermatitis was enterocolitis (28.0%) and
followed by common cold (8.0%), measles (8.0%) and pneumonia (6.0%).
4) The positive rate of KOH scraping was 31.3%, and the positive rate of culture on ordinary SGA media was 50.0%.
5) Most of patients belonged to grade 2 and it¢¥s positive rate of KOH scraping and culture were 67. 6%, 83.8% respectively.
6) To compare the positive rate of culture, the sick group showed remarkably higher rate than healthy group (65.5%:38.1%).
7) Among 75 candida species, Candida (C) albicans (88%), C paratropicalis (2.7%), C parapsilosis (2. 7%), C tropicalis (1.3%), C stellatoidea (1.3%), C guilliermondii (1.3%), and Cryptococcus (1.3%), were identified and 1 species could not be identified. C species encountered in grade 4 and buttock was only C albicans.
